Change Agent is a recognition program that enables people living with Lennox-Gastaut syndrome (LGS) to nominate people in their lives who have made a positive impact. The program aims to celebrate and honor the people whose positive actions and/or behavior benefit families living with LGS and promote acceptance, compassion, and kindness toward the LGS community at large.
A Change Agent can be almost anyone, within the following parameters:
Complete the online nomination form on this website. The person you nominate will receive an email from the Change Agent program once the nomination form is complete, so please have their email address ready to enter when you fill out the form.
Those Change Agents who are selected to be featured will be asked to provide their consent; otherwise, there is no additional action or obligation. While the people nominated did not seek this praise, simply agreeing to be recognized as a Change Agent helps to raise awareness about Lennox-Gastaut syndrome (LGS). Change Agents serve as examples of how compassion and thoughtful actions can be especially meaningful in the lives of people living with rare diseases, like LGS.
Anyone who is successfully nominated online will receive a congratulatory Change Agent certificate and the acknowledgement that he or she has made a difference in the life of someone who is living with Lennox-Gastaut syndrome (LGS). Additionally, each month a Change Agent will be selected to be featured on the Change Agent website and Facebook. Featured Change Agents will be contacted for their approval before being featured.
No. Participation in the Change Agent program is free.
